Waiver and Release from Liability Form

In consideration of my application and permitting me to participate in the Freediving, Spearfishing, or Snorkeling activities, I hearby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

  • I fully understand and accept that the activities offered have inherent dangers and risks to which I will be exposed by participating in and travelling to Costa Rica, including (but not limited to) property loss, serious injury and death.

  • I certify that I am physically fit and capable of participating in Freediving, Spearfishing, or Snorkeling activities and have not been advised otherwise by a qualified medical person.

  • I certify that there are no health-related reasons or problems which preclude my participation.

  • I have health cover/insurance that covers me for accident and injury in Costa Rica.

  • I acknowledge that this Accident Waiver and Release of Liability Form will be used by Altum Adventures SRL so that I may participate and that it will govern my actions and responsibilities.

  • I fully accept and assume all risks of participating in the activities and accept personal responsibility for any damages and expenses arising from my participation. I agree to adhere to and obey all, local Laws.

  • I acknowledge that this accident Waiver and Release from Liability Form is made for the benefit of Altum Adventures SRL, its employees, volunteers, and their respective successors and assigns (individually and collectively “Releasees”).

  • I release and discharge Releasees from any and all liability, and waive all claims, suits, and actions of any kind against Releasees, for death, disability, personal injury, property damage, theft, and any other loss or damages incurred that may hereafter accrue to me, my executors, administrators, heirs, next of kin, successors and assigns arising out of or in any way connected with my

    participation in the activities.

  • I will indemnify, defend and hold harmless any and all Releasees from any and all costs, losses, claims, liabilities, damages, or expenses incurred by such Releasees as a result of my actions or omissions during the activities.

    Photo Release

  • I agree to assign and grant the right and permission for Altum Adventures SRL to use and publish any photographs/film/video/electronic representations and/or sound recordings of myself on official communication channels.

  • I hereby release the Altum Adventures SRL from any and all liability from such use and promotion.

  • I hereby authorise the reproduction, sale, copyright, exhibit, broadcast, electronic storage and/or distribution of said photographs/film/video tapes/electronic representations and/or sound recordings without limitation at the discretion of Altum Adventures SRL .

  • I waive any rights, claims or interest and I understand that there will be no financial or other remuneration.

    I certify that I have fully read this document and fully understand its content and its implications. I fully agree to the terms of this Waiver/Release from Liability Form and

    Photo Release Form.

    The accident waiver, release of liability and image release shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. It shall not be modified in any way. If any part of this accident waiver and release from liability is determined to be invalid by law, all other parts of this waiver shall remain valid and enforceable.

    I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT ON MY OWN FREE WILL.
